الوصف: Developmental venous anomaly (DVA) is the most common type of intracranial vascular malformation. These lesions are benign and are considered to be non-pathological variants of normal deep parenchymal veins. Although most of them are asymptomatic, a small subset of them located in aqueductal region have been reported to cause obstructive hydrocephalus. The authors present an interesting case of biventricular hydrocephalus secondary to a DVA located on the proximal aqueduct in an adolescent patient. This case is discussed with in corroboration with current literature and management recommendations.

مستخلص: Abstract: The authors report a 49-year-old man who presented with obstructive hydrocephalus caused by a thrombosed posterior cerebral artery aneurysm. He underwent an endoscopic third ventriculostomy for the obstructive hydrocephalus followed by endovascular occlusion for the aneurysm. To our knowledge, this is the first report of a thrombosed posterior cerebral artery aneurysm presenting with obstructive hydrocephalus treated by the combination of endoscopic and endovascular techniques. مستخلص: Abstract: Among patients with idopathic aqueductal stenosis or impedance of cerebrospinal fluid (CSF) flow in the posterior fossa due to tumour, endoscopic fenestration of the floor of the third ventricle creates an alternative route for CSF flow to the subarachnoid space via the prepeduncular cistern. By reestablishing CSF flow, this procedure dissipates any pressure gradient on midline structures. This may obviate the need for traditional CSF shunt diversion techniques in such settings. Currently, endoscopic third ventriculostomy is indicated in approximately 25% of patients with hydrocephalus and can be performed instead of shunt placement. Appropriate patients are those with aqueductal stenosis (10%), obstructive tumours (10%), and obstructive cysts (5%). Additional recent data suggest the favorability of third ventriculostomy over shunt implantation in additional patient cohorts. Operative technique is discussed. [Copyright &y& Elsevier]

الوصف: A middle-aged man presented with syncope and confusion. Neuroimaging revealed a third ventricular mass with obstructive hydrocephalus and bilateral convexity meningiomata. The masses were excised and pathology showed a colloid cyst and WHO grade 1 meningiomata respectively. Multisystem workup confirmed Gorlin syndrome. To our knowledge, this is the fourth reported case of Gorlin syndrome associated with colloid cyst, and the first case where multiple meningiomata are also present.

الوصف: Intraventricular schwannomas are rare, with only 20 cases reported in the literature to our knowledge, including two cases of schwannomas in the third ventricle. We report a 23-year-old man with a mass in the posterior portion of the third ventricle and obstructive hydrocephalus. The intraventricular mass was clearly visualized and removed by neuroendoscopic surgery, and endoscopic third ventriculostomy was simultaneously performed for the hydrocephalus. Histological and immunohistochemical assays of the surgical sample established a diagnosis of intraventricular schwannoma. The patient had a favourable outcome and no neurological deficits 1 year later. The schwannoma was distant from the choroid plexus, which supports the hypothesis that the schwannoma originated from ectopic neural crest cells that were displaced during embryogenesis.

الوصف: Xanthogranuloma of the sellar region is rare. It typically causes symptoms of pituitary dysfunction or visual change due to compression of the optic chiasm. However, to the best of our knowledge, no case of xanthogranuloma-associated hydrocephalus has been documented in the literature. This study describes a 32-year-old man afflicted with xanthogranuloma of the sellar region who presented with acute change in consciousness and associated obstructive hydrocephalus. A literature review and discussion regarding therapeutic options for this condition are presented.

مستخلص: Abstract: Reversible posterior leukoencephalopathy syndrome (RPLS) is characterized clinically by headache, altered mental status, abnormal visual perception and seizures. It is associated with neuroradiological findings characterized by reversible white matter abnormalities, predominantly in the parietal-occipital areas. Since the first description in 1996, it has been recognized in an increasing number of medical conditions, including hypertensive encephalopathy, eclampsia, and immunosuppressive treatment. The rapid resolution of clinical and neuroradiologic abnormalities suggests cerebral oedema, which is thought to result from impaired cerebrovascular autoregulation and endothelial injury. We report a patient presenting with acute onset progressively worsening headache and confusion associated with uncontrolled hypertension. CT and MRI revealed acute non-communicating hydrocephalus secondary to cerebellar and pontine oedema. The patient became drowsy, so an external ventricular drain was inserted for decompression of the acute hydrocephalus, and his blood pressure was aggressively managed. The patient recovered well with complete clinical and radiological resolution. This case illustrates the reversibility of RPLS if it is diagnosed early and appropriate treatment is instituted. الوصف: The rosette-forming glioneuronal tumor (RGNT) of the fourth ventricle is a recently described, rare, and distinct tumor of the glioneuronal family. The presentation, natural history, and treatment response of these tumors has been unclear as there are no significant series of a sizeable population with long-term follow-up. We report a comprehensive analysis of 41 patients with RGNT to provide the most current understanding of this rare tumor. Treatment of these patients has consisted of resection via the transvermian and telovelar approaches, with one patient requiring radiotherapy due to tumor recurrence. Various unique imaging characteristics may allow for the preoperative identification of these tumors. Resection via the telovelar approach should be considered for symptomatic tumors and those that pose a risk of obstructive hydrocephalus. Due to their benign nature and low propensity for recurrence, subtotal resection may be appropriate for those that are adherent to the brainstem. Radiotherapy may be considered for patients with tumor recurrence.
