Introduction and Objective: Tubularisation of urethral plate is common practice for Urethroplasty. But in cases with Bad plate, tubularisation may decrease the blood supply to the plate due to detachment from thin subcutaneous tissue. Buried epithelial strip in hypospadias was conceptualized by Dupley in 1880 and Marion in 1922 and popularized by Denis Browne in 1949. Same principle was adopted in this study along with Tunica Vaginalis as a wrap to increase the vascularity around urethral plate. Materials and Methods: Out of 22 patients since Jan 2005. Nov 2006, four had no plate at all and rest of the patients had narrow (1-2 mm) urethral plate in the penile portion. 7 patients had DISTAL penile, 9 had mid-penile and 6 had proximal penile Hypospadias. Age range 2 1/2 - 22 (mean 4.7) years. U-shaped incision were made around meati and 'supposed to be urethral plate'. Degloving was done and margins of 'supposed to be plate' were fixed to the surrounding tissues by delayed absorbable suture materials. After putting an appropriate sized catheter, tunica vaginalis wrapped around plate of urethra without making it a tube. All of the patients were advised regular neo-urethral dilation in follow up. Results: Mean (range) follow up period was 13.8 (8-24) months. The result is good in twenty patients. Suture tract fistula = 5, Fistula = 1, wound dehiscence = 1, Extravasations = 1. Suture tract fistulas were cured by regular dilation and extravasations was managed by per-urethral catheterization. Two patients (9%) needed second operation for repair of fistula and wound dehiscence. Conclusion: Making the tube with bad plate akin to skin seemed to be risky hence; this modified Browne's procedure was adopted. It is a simple procedure with a good surgical outcome.
