Tumour necrosis factor receptor-associated periodic syndrome (TRAPS), a rare autosomal dominant disorder, is characterised by recurrent attacks of fever, myalgias, and abdominal pain. However, manifestations in the central nervous system are hardly known. We describe a family in which one of three affected members developed central nervous system symptoms. First diagnosed as multiple sclerosis (MS), the demyelinisation seems to be a feature of TRAPS rather than MS. The syndrome is discussed as a rare differential diagnosis of MS. Das TNF-Rezeptor-assoziierte periodische Syndrom (TRAPS), ein seltenes hereditäres Fiebersyndrom, ist durch rezidivierende Fieberepisoden mit Myalgien, abdominalen Schmerzen mit Koliken und Exanthemen gekennzeichnet. Symptome des ZNS wurden bei einzelnen TRAPS-Patienten beschrieben; ob es sich hierbei um eine Manifestation im Rahmen des Fiebersyndroms handelt, ist noch unklar. Bei einer Patientin mit TRAPS traten schubweise ZNS-Symptome auf, die zunächst zur Diagnose einer Multiplen Sklerose führten. Erst durch den Verlauf entstand der Verdacht, dass es sich hier eher um eine ZNS-Manifestation des TRAPS handelt, welches damit als sehr seltene Differenzialdiagnose der Multiplen Sklerose diskutiert wird.
