    الوصف: Disease modification has become a well-established concept in several therapeutic areas; however, no widely accepted definition of disease modification exists for SLE. We reviewed established definitions of disease modification in other conditions and identified a meaningful effect on ‘disease manifestations’ (ie, signs, symptoms and patient-reported outcomes) and on ‘disease outcomes’ (eg, long-term remission or progression of damage) as the key principles of disease modification, indicating a positive effect on the natural course of the disease. Based on these findings and the treatment goals and outcome measures for SLE, including lupus nephritis, we suggest a definition of disease modification based on disease activity indices and organ damage outcomes, with the latter as a key anchor. A set of evaluation criteria is also suggested. Establishing a definition of disease modification in SLE will clarify which treatments can be considered disease modifying, provide an opportunity to harmonise future clinical trial outcomes and enable comparison between therapies, all of which could ultimately help to improve patient outcomes. This publication seeks to catalyse further discussion and provide a framework to develop an accepted definition of disease modification in SLE.

    الوصف: Aim Seasonally dry tropical forest (SDTF) of the Caribbean Islands (primarily West Indies) is floristically distinct from Neotropical SDTF in Central and South America. We evaluate whether tree species composition was associated with climatic gradients or geographical distance. Turnover (dissimilarity) in species composition of different islands or among more distant sites would suggest communities structured by speciation and dispersal limitations. A nested pattern would be consistent with a steep resource gradient. Correlation of species composition with climatic variation would suggest communities structured by broad-scale environmental filtering. Location The West Indies (The Bahamas, Cuba, Hispaniola, Jamaica, Puerto Rico, U.S. Virgin Islands, Guadeloupe, Martinique, St. Lucia), Providencia (Colombia), south Florida (USA), and Florida Keys (USA). Taxon Seed plants -- woody taxa (primarily trees). Methods We compiled 572 plots from 23 surveys conducted between 1969-2016. Hierarchical clustering of species in plots, and indicator species analysis for the resulting groups of sites, identified geographical patterns of turnover in species composition. Non parametric analysis of variance, applied to principal components of bioclimatic variables, determined the degree of covariation of climate with location. Nestedness versus turnover in species composition was evaluated using beta diversity partitioning. Generalized dissimilarity modeling partitioned the effect of climate versus geographical distance on species composition. Results Despite a set of commonly occurring species, SDTF tree community composition was distinct among islands and was characterized by spatial turnover on climatic gradients that covaried with geographical gradients. Greater Antillean islands were characterized by endemic indicator species. Northern subtropical areas supported distinct, rather than nested, SDTF communities in spite of low levels of endemism.     الوصف: Dinitrogen pentoxide (N2O5) plays a central role in nighttime tropospheric chemistry as its formation and subsequent loss in sink processes limits the potential for tropospheric photochemistry to generate ozone the next day. Since accurate observational data for N2O5 are critical to examine our understanding of this chemistry, it is vital also to evaluate the capabilities of N2O5 measurement techniques through the co-deployment of the available instrumentation. This work compares measurements of N2O5 from two aircraft instruments on board the Facility for Airborne Atmospheric Measurements (FAAM) BAe-146 aircraft during the Role of Nighttime Chemistry in Controlling the Oxidising Capacity of the Atmosphere (RONOCO) measurement campaigns over the United Kingdom in 2010 and 2011. A chemical ionisation mass spectrometer (CIMS), deployed for the first time for ambient N2O5 detection during RONOCO, measured N2O5 directly using I- ionisation chemistry and an aircraft-based broadband cavity enhanced absorption spectrometer (BBCEAS), developed specifically for RONOCO, measured N2O5 by thermally dissociating N2O5 and quantifying the resultant NO3 spectroscopically within a high finesse optical cavity. N2O5 mixing ratios were simultaneously measured at 1 second time resolution (1 Hz) by the two instruments for 8 flights during RONOCO. The sensitivity for the CIMS instrument was 52 ion counts per pptv with a limit of detection of 7.4 pptv for 1 Hz measurements. BBCEAS, a proven technique for N2O5 measurement, had a limit of detection of 2 pptv. Comparison of the observed N2O5 mixing ratios show excellent agreement between the CIMS and BBCEAS methods for the whole dataset, as indicated by the square of the linear correlation coefficient, R-2 = 0.89. Even stronger correlations (R2 values up to 0.98) were found for individual flights. Altitudinal profiles of N2O5 obtained by CIMS and BBCEAS also showed close agreement (R-2 = 0.93).     الوصف: Population-based, person-specific health surveys, with concomitant biological measures, should provide important information about the processes by which socioeconomic and psychosocial factors embed themselves in human health. Questionnaire responses allow for assessment of the perceived psychosocial environment, but biological measurements will measure the status of the psychoneuroimmunology/psychoneuroendocrinology (PNI/PNE) pathways and may allow us to identify people who have 'adapted' to their stress because of experience, expectations, stoicism, etc. This review sets criteria to evaluate potential physiological markers of chronic stress. Because population health surveys involve a massive number of samples, special consideration must be given to the laboratory analysis method and transportation time of the markers chosen. We reviewed five areas: glycosylated proteins, the immune system, hemostasis, peripheral benzodiazepine receptors, and the waist-hip ratio.
