随着人民生活水平的提高、人口的老年化、生活方式的改变，我国非传染性慢性病的危害将呈持续上升的趋势。目前尚未掌握完全治愈糖尿病的方法，应加强对人群的卫生健康知识教育，尽早干预不良的生活方式，促成健康生活方式的养成，减少各种慢性病的发病率，控制病情的发展，提高人民的生活质量。 目前，社会工作介入慢性病自我管理的干预的研究还比较少。本研究采用质性研究方法。本文通过对X市一家社区卫生服务中心开展的糖尿病自我管理项目——“糖尿病病友全程关照网”进行个案研究，分析社会工作者在解决项目中存在问题的过程中，所发挥的角色作用和对项目的影响。 研究发现，社工在多项目合作中主要承担了协调者、组织者、资源筹措者、. ; With the improvement of people's living standard, the population’s aging, the change of life style, the harm from infectious chronic disease in China will continued to rise.
