    الوصف: This policy analysis investigates the benefits of full practice authority (FPA) for Nurse Practitioners (NPs) in the Southwestern United States, using Arizona as an FPA model and California and Tennessee as comparison states. The PICO question that directed the project is: for NPs practicing as advanced practice nurse practitioners within the Southwestern United States region (P), does a comprehensive policy recommendation for FPA for NP-led clinics in the secondary education space (I) compared to existing variable approaches within the region (C) improve access to care and direct or indirect patient outcomes as endorsed by the literature (O)? A comprehensive literature review revealed that the United States is experiencing a significant primary care provider shortage. Although NPs are educationally prepared, tested, certified, and licensed to provide comprehensive patient care, California and Texas continue regulatory restrictions on NP scope of practice. These practice restrictions exacerbate the care shortage, especially in rural and primary health professional shortage areas. Furthermore, the literature search revealed consistent support of FPA by critical stakeholders, such as the American Association of Nurse Practitioners, the American Nurses Association, and the Consensus Model. Policy briefs advocating for evidence-based recommendations for FPA are presented for dissemination to the legislative bodies, professional organizations, and the state boards of nursing. Eliminating practice barriers allows NPs to practice to the fullest extent of their education and training. This results in greater access to care, reduced healthcare costs, increased quality of care, and works to lessen health disparities.

    الوصف: Practice Problem: The practice problem is a patient navigation tool in a mobile health clinic for the Latino population to help improve utilization of resources within Daviess County, Indiana, and improve overall health literacy and health of those populations? PICOT: The PICOT question that guided this project was: In a mobile health clinic serving Latino populations (P), does a navigation tool (I) compared to current practices (C) improve utilization of services (O) within a 12-week timeframe (T)? Evidence: The evidence that guided this project showed patients with limited English proficiency, immigrant families have a disparity of health literacy, and an increased rate of social needs have a low utilization of community resources. Intervention: The intervention will provide a navigation tool containing resource contacts for health and social needs for the community that will be translated into Spanish and Haitian Creole languages to be distributed at a mobile health clinic. Outcome: The resources associated with food assistance and requests for education in stretching food dollars showed an increase in utilization by the Latino populations in the community. The vaccines administered by the health department did not show significant improvement. Conclusion: This DNP project can help to increase health equity and overall health in Latino populations by offering a comprehensive list of resources in a navigational tool and an emergency list of resources on a magnet that is easily visible. This provides the population knowledge on how to contact and find the resources, as well as the assistance those resources can provide.

    الوصف: Despite an increase in healthcare expenditure, racial and ethnic disparities remain a substantial concern among adult primary care patients in the United States. This is largely due to the focus on medical intervention and disregard for the underlying factors that contribute significantly to health outcomes such as economic stability, educational access and quality, healthcare access and quality, neighborhood and built environment, and social and community context. The PICOT question that guided this program evaluation review project is: In adult clients seeking care at a primary care clinic (P), how does a SDOH toolkit based on the CDC framework (I) compared to no SDOH toolkit (C) affect identification of unmet health related social needs (O)? The evidence suggests that screening for SDOH identifies unmet social needs, improves provider referrals to relevant resources, and improves overall health outcomes. In addition, the literature supports the use and development of nursing toolkits to influence evidence-based interventions in healthcare. A detailed and thorough program evaluation review of the Centers for Disease Control and Prevention, Healthy People 2030, and the Health Leads Network SDOH programs revealed that the evaluated SDOH programs meet the requirements of a program per the CDC Program Evaluation framework. Additionally, the evidence supported the use of these programs as established to support the development of an SDOH screening toolkit for the specialty population of adult primary care patients. Screening for SDOH in adult primary care is a necessary component for improving health outcomes and use of an SDOH screening toolkit in the clinical practice setting will help assist in the smooth and successful implementation of SDOH screening for all adult primary care patients.
