| الوصف: | Aims: To describe the process of working with professionals of primary care, as regards the production and use of health records; discuss the importance of information management in health and its relevance to the quality of nursing records. Method: Exploratory-descriptive study with a qualitative approach, field research to document analysis that took place at three municipal centers, where live family health teams with other unit staff. The theoretical framework is based on the concept of reflective practice advocated by Donald Schön that emphasizes education and learning by doing. Twelve workers were active in the units setting of the research were interviewed and the data analyzed from the dimensions for analysis and attributes for quality management of information proposed by De Sordi. Results: Three categories emerged from the study, which were analytically named: 1) The practice of registration in the “doing” of nursing; 2) the quality of the record in the thinking of nursing; and 3) the nursing records in the context of health information management. Conclusion: That it is necessary to invest in lifelong learning strategies to promote the development of an intellectual responsibility and consequently the improvement of the practice of recording in nursing. ; Objetivos: Describir el proceso de trabajo de los profesionales de atención primaria con relación a la producción y utilización de los registros en salud y discutir la importancia de la gestión de la información en salud y su pertinencia para la calidad de los registros de enfermería. Método: Estudio exploratorio-descriptivo, con planteamiento cualitativo; investigación de campo con análisis documental que tuvo como escenarios tres centros municipales de salud ubicados en la ciudad de Río de Janeiro, Brasil.
