Introducción: el hematoma subdural crónico, como antecedente, es una de las patologías neuroquirúrgicas más frecuentes.Objetivo: describir la fisiopatología y terapéutica actual del hematoma subdural crónico.Métodos: se realizó una revisión bibliográfica de tipo descriptiva sobre el hematoma subdural crónico durante los meses de marzo a abril del 2023. Se emplearon las bases de datos SciELO, Scopus y PubMed. Se seleccionaron 15 artículos. Resultados: el diagnóstico oportuno y sobre todo la terapéutica adecuada puede mejorar mayormente el pronóstico de los pacientes. La etiología más frecuente es la traumática; sin embargo, no se pueden dejar de lado algunas comorbilidades y sus tratamientos como la anticoagulación; situaciones que juegan un papel importante para favorecer la formación de dicha patología. La atorvastatina demostró ser muy útil en pacientes que no requieren cirugía a brevedad.Conclusiones: el hematoma subdural crónico es una patología neuroquirúrgica de elevada frecuencia en la práctica de medicina general y en la especialidad de neurocirugía. Existen dos teorías que explican la formación de los hematomas subdurales crónicos. La cirugía se mantiene como el manejo de elección; sin embargo, existen fármacos como las estatinas, especialmente la atorvastatina, de gran valor terapéutico. ; Introduction: chronic subdural hematoma is one of the most frequent neurosurgical pathologies.Objective: to describe the pathophysiology and current therapy of chronic subdural hematoma.Methods: a descriptive literature review on chronic subdural hematoma was carried out from March to April 2023. The SciELO, Scopus and PubMed databases were used. Fifteen articles were selected.Results: timely diagnosis and, above all, adequate therapy can greatly improve the prognosis of patients.
