الوصف: Incidence of type 2 diabetes mellitus (T2DM) has been continually rising during the past years as a result of sedentary lifestyle and increasing prevalence of obesity. Available therapeutic options for disease management are constantly expanding by the addition of new pharmacologic categories with pleiotropic actions beyond glycaemic control. In this regard glucagon-like peptide 1 (GLP-1) receptor agonists such as liraglutide are injectable antidiabetic therapies with side benefits for patients because they reduce body weight as well as risk of cardiovascular adverse events in patients with established atherosclerotic disease. In addition, they confer renoproctive properties by ameliorating progression of albuminuria. Patients with type 2 diabetes suffer from many comorbidities amongst which arterial hypertension is highly prevalent and associated with increased incidence of cardiovascular events and mortality. Aim of the present thesis is to assess the effect of liraglutide, a long acting GLP-1 receptor agonist, on blood pressure of hypertensive patients with type 2 diabetes by means of a double blind, randomised controlled trial. Patients with inadequate glycaemic control (haemoglobin A1c 7-10%) and pre-hypertension or stage 1 hypertension were deemed eligible and were randomised to five-week treatment with liraglutide or placebo. To ascertain the effect of liraglutide on blood pressure and heart rate 24-h ambulatory monitoring was utilised, whilst a parallel mechanistic study was developed to elucidate the effect of liraglutide on renal sodium handling. Sixty two patients with a mean age of 60.2 years participated in the trial and were equally randomised to liraglutide and placebo arms. All participants received background therapy with metformin, whilst 35.5% and 14.5% of them received concomitant therapy with sulphonylurea and pioglitazone respectively.
