    الوصف: The emergence of 5G technologies opens up new opportunities for railway communications. One of the foundational aspects of 5G architecture is its control-plane programmability, which can be achieved through Software Defined Networking (SDN). In railway scenarios, this can be used to dynamically reconfigure the network for a more effective and efficient management of communication flows produced by moving trains. The paper presents a framework for integrating modelling and analysis tools into a programmable control plane specifically tailored to railway communications. We introduce the concept of domain-awareness in the network control plane as an SDN-enabled feature that allows achieving application-specific advantages besides those purely expressed in terms of key performance indicators such as the quality of service. We propose a reference architecture in which domain-awareness in the control plane is obtained by considering information gathered by network devices and ad-hoc communication gateways that are able to detect relevant signalling events. In the architecture, the actual behaviour of the SDN controller is governed by applications that are able to react to specific triggers and re-configure network devices accordingly. We also provide a methodological framework based on model-driven engineering and formal methods, including dynamic state machines, for the automatic generation of SDN control plane logic.

    الوصف: Within Artificial Intelligence, Deep Learning (DL) represents a paradigm that has been showing unprecedented performance in image and audio processing by supporting or even replacing humans in defect and anomaly detection. The Railway sector is expected to benefit from DL applications, especially in predictive maintenance applications, where smart audio and video sensors can be leveraged yet kept distinct from safety-critical functions. Such separation is crucial, as it allows for improving system dependability with no impact on its safety certification. This is further supported by the development of DL in other transportation domains, such as automotive and avionics, opening for knowledge transfer opportunities and highlighting the potential of such a paradigm in railways. In order to summarize the recent state-of-the-art while inquiring about future opportunities, this paper reviews DL approaches for the analysis of data generated by acoustic and visual sensors in railway maintenance applications that have been published until August 31st, 2021. In this paper, the current state of the research is investigated and evaluated using a structured and systematic method, in order to highlight promising approaches and successful applications, as well as to identify available datasets, current limitations, open issues, challenges, and recommendations about future research directions.

    الوصف: Italy is a country with high seismic hazard, however since the delay in the seismic classification of the national territory, most of the existing building heritage does not comply with the current technical standards for buildings. The seismic events that have hit different Italian regions in recent years have highlighted the complexity of the challenge for the public bodies both in the emergency management and post‐event reconstruction and in the planning of effective risk prevention and mitigation measures to be implemented in ‘peacetime’. These difficulties concern, in particular, the capacity to properly manage the financial and technical resources available and to identify the intervention priorities throughout the entire emergency cycle. For correct management, the priority is to quantify and localize, through simulations, the quantification of probable damages and to evaluate in terms of cost‐benefits the possible alternative strategies for mitigation, also taking into account the potential, in terms of cost‐effectiveness, of integrated measures for seismic and energy retrofitting. In this framework, the project CAESAR II (Complementary Analyses for Emergency planning based on Seismic Risks impact evaluations) has been developed as a Decision Support System for Public Authorities in charge of developing Disaster Risk Reduction plans, with the possibility of programming mid to long‐term investments for public and private properties, as well as defining custom financial support mechanisms and tax incentives.

    الوصف: This study reports the data on the contamination caused by polycyclic aromatic hydrocarbons (PAHs) drained from the Volturno River. The seasonal and spatial distribution of PAHs in water and sediment samples was assessed. The 16 PAHs were determined in the water dissolved phase (DP), suspended particulate matter (SPM), and sediments. A multidimensional statistical approach was used to identify three pollution composite indicators. Contaminant discharges of PAHs into the sea were calculated in about 3158.2 kg/year. Total concentrations of PAHs varied in ranges 434.8 to 872.1 ng g−1 and 256.7 to 1686.3 ng L−1 in sediment samples and in water (DP + SPM), respectively. The statistical results indicated that the PAHs mainly had a pyrolytic source. Considering the sediment quality guidelines (SQGs), the water environmental quality standards (USEPA EQS), and risk quotient (RQ), the Volturno River would be considered as an area in which the environmental integrity is possibly at risk.

    الوصف: The gut microbiota may have an impact on obesity. To date, the majority of studies in obese patients reported microbiota composition in stool samples. The aim of this study was to investigate the duodenal mucosa dysbiosis in adult obese individuals from Campania, a region in Italy with a very high percentage of obese people, to highlight microbial taxa likely associated with obesity. Duodenum biopsies were taken during upper gastrointestinal endoscopy in 19 obese (OB) and 16 lean control subjects (CO) and microbiome studied by 16S rRNA gene sequencing. Duodenal microbiome in our groups consisted of six phyla: Proteobacteria, Firmicutes, Actinobacteria, Fusobacteria, Bacteroidetes and Acidobacteria. Proteobacteria (51.1% vs. 40.1%) and Firmicutes (33.6% vs. 44.9%) were significantly (p < 0.05) more and less abundant in OB compared with CO, respectively. Oribacterium asaccharolyticum, Atopobium parvulum and Fusobacterium nucleatum were reduced (p < 0.01) and Pseudomonadales were increased (p < 0.05) in OB compared with CO. Receiver operating characteristic curve analysis showed Atopobium and Oribacterium genera able to discriminate with accuracy (power = 75% and 78%, respectively) OB from CO. In conclusion, increased Proteobacteria and decreased Firmicutes (Lachnospiraceae) characterized the duodenal microbiome of obese subjects. These data direct to further studies to evaluate the functional role of the dysbiotic-obese-associated signature.

    الوصف: Objective: This study was undertaken to assess the impact of immunosuppressive and immunomodulatory therapies on the severity of coronavirus disease 2019 (COVID-19) in people with multiple sclerosis (PwMS). Methods: We retrospectively collected data of PwMS with suspected or confirmed COVID-19. All the patients had complete follow-up to death or recovery. Severe COVID-19 was defined by a 3-level variable: mild disease not requiring hospitalization versus pneumonia or hospitalization versus intensive care unit (ICU) admission or death. We evaluated baseline characteristics and MS therapies associated with severe COVID-19 by multivariate and propensity score (PS)-weighted ordinal logistic models. Sensitivity analyses were run to confirm the results. Results: Of 844 PwMS with suspected (n = 565) or confirmed (n = 279) COVID-19, 13 (1.54%) died; 11 of them were in a progressive MS phase, and 8 were without any therapy. Thirty-eight (4.5%) were admitted to an ICU; 99 (11.7%) had radiologically documented pneumonia; 96 (11.4%) were hospitalized. After adjusting for region, age, sex, progressive MS course, Expanded Disability Status Scale, disease duration, body mass index, comorbidities, and recent methylprednisolone use, therapy with an anti-CD20 agent (ocrelizumab or rituximab) was significantly associated (odds ratio [OR] = 2.37, 95% confidence interval [CI] = 1.18-4.74, p = 0.015) with increased risk of severe COVID-19. Recent use (<1 month) of methylprednisolone was also associated with a worse outcome (OR = 5.24, 95% CI = 2.20-12.53, p = 0.001). Results were confirmed by the PS-weighted analysis and by all the sensitivity analyses. Interpretation: This study showed an acceptable level of safety of therapies with a broad array of mechanisms of action. However, some specific elements of risk emerged. These will need to be considered while the COVID-19 pandemic persists.

    الوصف: Background: Treatment targets of ulcerative colitis (UC) have evolved to include not only endoscopic but also histologic remission. However, the concept of histological activity is still in its early days. We aimed to capture the attitudes toward UC histology and the uptake of standardized reporting of endoscopy and histology of UC in daily practice. Methods: We conducted a cross-sectional survey of physicians involved in the care of inflammatory bowel disease worldwide. The survey included 21 questions divided into three sections. The first recorded demographics, specialty, and level of experience of participants; the second covered clinical practices and attitudes toward the use and reporting of endoscopy; and the third covered histology. Results: In total, 359 participants from 60 countries and all levels of experience completed the survey. UC histology was used by nearly all respondents (90.5%) for initial diagnosis, by 72% to monitor disease course, by 62.4% to determine the microscopic extension, by 59.9% to confirm deep remission when considering to stop treatment, and 42.3% to increase/optimize treatment. Nevertheless 77.2% of participants reported that no standard histological index was available in their daily practice. Instead, endoscopy reports included the Mayo Endoscopic score in 90% of cases. The majority of respondents welcomed as useful or very useful an artificial intelligence system to automate scoring of endoscopy (69%) or histology (73%). Conclusion: UC histology reports are less standard than endoscopy reports, although most physicians consider histological activity useful when managing UC and would welcome artificial intelligence systems to automate endoscopic and histological scoring.

    الوصف: Backgrounds and aims: Absence of neutrophils is the minimum standard to consider histological remission of ulcerative colitis (UC). The PICaSSO Histological remission Index (PHRI) is a new simple index for UC based only on the detection of neutrophils. We evaluate PHRI's correlation with endoscopy and its prognostic value compared to other established indices. Methods: Consecutive patients with UC underwent colonoscopy at 2 referral centres (Birmingham, UK and Milan, Italy) and were followed up for 2 years. Correlation between histology (PHRI, Nancy [NHI] and Robarts [RHI]) and endoscopy (Mayo Endoscopic Score [MES]; Ulcerative Colitis Severity Endoscopic Index of Severity [UCEIS], and PICaSSO score) was calculated as Spearman coefficients. Diagnostic performance of endoscopy was assessed with ROC curves and outcome stratification with Kaplan-meier curves. Results: 192 patients with UC were enrolled, representing all grades of endoscopic severity. Correlation between histology and endoscopy did not differ significantly when using PHRI instead of NHI or RHI. In particular, PHRI's correlation with MES, UCEIS and PICaSSO was 0.745, 0.718, and 0.694, respectively. Endoscopically-assessed remission reflected the absence of neutrophils (PHRI = 0) with areas under the ROC curve of 0.905, 0.906, and 0.877 for MES, UCEIS, and PICaSSO, respectively. The hazard ratio for disease flare between patients in histological activity/remission was statistically similar (p>0.05) across indexes (2.752, 2.706, and 2.871 for RHI, NHI, and PHRI, respectively). Conclusion: PHRI correlates with endoscopy and stratifies risk of relapse similarly to RHI and NHI. Neutrophil-only assessment of UC is a simple yet viable alternative to established histological scores.
