    الوصف: Nitrogen (N) is one of the most essential nutrients needed to reach maximum grain yield in all environments. Nitrogen fertilizers represent an important production cost, in both monetary and environmental terms. The aim of this study was to assess the effect of preplant nitrogen (N) rate and topdress N source on spring wheat (Triticum aestivum L.) grain yield and quality. Study was conducted in North-Central and Western Montana from 2011 to 2013 (total of 6 site-years). Six different preplant nitrogen (N) rates (0, 220, 22, 44, 67, and 90 N rate, kg ha−1) followed by two topdress N sources (urea, 46-0-0, and urea ammonium nitrate (UAN), 32-0-0) were applied to spring wheat (Triticum aestivum L.). The results showed that there were no significant differences in grain yield, protein content, or protein yield, associated with topdress N source.

    الوصف: Experiments were conducted in Montana to evaluate Environmentally Smart Nitrogen (ESN) as a nitrogen (N) source in wheat. Plots were arranged in a split-plot design with ESN, urea, and a 50%-50% urea-ESN blend at low, medium, and high at-seeding N rates in the subplot, with four replications. Measurements included grain yield (GY), protein (GP), and N uptake (GNU). A partial budget economic analysis was performed to assess the net benefits of the three sources. Average GY varied from 1816 to 5583 kg ha−1 and grain protein (GP) content ranged from 9.1 to 17.3% among site-years. Urea, ESN, and the blend resulted in higher GYs at 3, 2, and 2 site-years out of 8 evaluated site-years, respectively. Topdressing N improved GY for all sources. No trend in GP associated with N source was observed. With GP-adjusted revenue, farmer would not recover investment costs from ESN or blend compared with urea. With ESN costing consistently more than urea per unit of N, we recommend urea as N source for spring wheat in Northern Great Plains.

    الوصف: An adrenal “incidentaloma” is defined as an unexpected finding on radiological imaging performed for unrelated indications. Improvements in radiological technology have seen a dramatic increase in this phenomenon. We report the unique case of a 60-year-old female presenting with a 6-month history of abdominal pain, altered bowel habit, and rectal bleeding. Her past medical history included situs inversus totalis and a patent ductus arteriosus. Colonoscopy revealed an ulcerated tumour in her sigmoid colon. Staging PET-CT confirmed a sigmoid tumour and also identified a large heterogenous enhancing FDG-avid right adrenal mass. Biochemical testing/MIBG imaging confirmed a right adrenal phaeochromocytoma. Hypertension was controlled and excision was performed via a transperitoneal laparoscopic adrenalectomy, in the left lateral decubitus position. Uniquely, liver retraction was not required due to its position in the left hypochondrium. Histology confirmed a benign 46 mm phaeochromocytoma. Subsequent uncomplicated sigmoid colectomy/right salpingo-oophorectomy for a locally advanced colonic tumour was performed with adjuvant chemotherapy. This case highlights the importance of accurately identifying functioning adrenal tumours before elective surgery as undiagnosed phaeochromocytomas carry significant intraoperative morbidity/mortality. Right adrenalectomy was made easier in this patient by the liver’s unique position. Uncomplicated colorectal resection was made possible by combined preoperative functional/anatomical imaging.

A multifactorial decision support system (mDSS) is a tool designed to improve the clinical decision-making process, while using clinical inputs for an individual patient to generate case-specific advice. The study provides an overview of the literature to analyze current available mDSS focused on prostate cancer (PCa), in order to better understand the availability of decision support tools as well as where the current literature is lacking. Methods. We performed a MEDLINE literature search in July 2018. We divided the included studies into different sections: diagnostic, which aids in detection or staging of PCa; treatment, supporting the decision between treatment modalities; and patient, which focusses on informing the patient. We manually screened and excluded studies that did not contain an mDSS concerning prostate cancer and study proposals. Results. Our search resulted in twelve diagnostic mDSS; six treatment mDSS; two patient mDSS; and eight papers that could improve mDSS. Conclusions. Diagnosis mDSS is well represented in the literature as well as treatment mDSS considering external-beam radiotherapy; however, there is a lack of mDSS for other treatment modalities. The development of patient decision aids is a new field of research, and few successes have been made for PCa patients. These tools can improve personalized medicine but need to overcome a number of difficulties to be successful and require more research.
