الوصف: Objective: after induction chemotherapy for head and neck cancer, CT is used to assess tumor response but lacks rationalized methods for measurement. In T3 laryngeal cancer, vocal cord remobilization is a major sign of tumor response. We compared the performances of RECIST 1.1, WHO and volumetric methods of evaluating response with laryngeal remobilization. Materials and Methods: neck CT-scans were acquired before and after induction chemotherapy from patients treated for T3 laryngeal cancer with initial vocal cord fixation. Tumor size was assessed according to RECIST 1.1, WHO, and volumetric methods. Tumor shrinkage on CT was compared to vocal cord remobilization. Areas under the curve (AUC) of ROC curves were compared and sensitivity and specificity of optimal cutoffs for CT prediction of vocal fold remobilization were determined. Results: Forty-three patients were included; vocal cord remobilization was observed in 24/43 after chemotherapy. AUCs were 0.763 (95%CI [0.615; 0.911]) for RECIST 1.1, 0.779 (95%CI [0.63; 0.971]) for WHO and 0.763 (95%CI [0.623; 0.904]) for volumetric evaluations with no significant difference. Optimal cutoffs for prediction of vocal fold remobilization were - 29% (Se = 70.8%; Sp =68.4%), - 47% (Se = 91.7%; Sp = 63.2%) and - 64% (Se = 75%; Sp = 63.2%) for RECIST 1.1, WHO and volumetric evaluation respectively. Conclusion: RECIST 1.1, WHO or volumetric measurements with CT-scanner match with vocal cord remobilization after induction chemotherapy in patient with T3 laryngeal cancer.
